Call to schedule vaccine appointment (copayments may apply - insurance depending)

Print and fill out Vaccine Screening & Consent Form (bring with you to the pharmacy).

Bring Medicare card or other insurance information.

Arrive at pharmacy and notify staff to confirm the vaccines you would like to receive. Review your Vaccine Information Statement.

Find a comfortable seat, ensure to relax the arm you would like to receive vaccination.

Vaccine will be administered by pharmacist.

Stay seated for 5-15min post vaccination to ensure no adverse reactions.
